Appoint for Voting Proxy Form - If a qualified elector is unable to vote on Election Day or on any of the advance voting days, he/she may appoint another qualified elector. Please read the instructions on the form carefully. The appointed individual must appear at the City Clerk's office, 545 Talbot Street, St. Thomas.
