Birth registrations are submitted electronically and directly to Service Ontario. Application for a birth certificate is made directly to the Office of the Registrar General at the address listed above, along with the appropriate fee. Applications for birth certificates may be obtained at the Clerk's Office, 1st floor, City Hall, 545 Talbot Street, St. Thomas or downloaded from the Ministry's website.
