Application should be submitted to the Office of the Registrar General at the address listed above, or online using the Death Certificate application form. Blank applications can also be obtained in person from the City Clerk's office, 1st Floor, City Hall, 545 Talbot Street, St. Thomas.

For deaths that occurred more than 70 years ago, please contact the Archives of Ontario.
